Required Forms

  • Effective July 1, 2026, all first-time high school participants in FHSAA athletics must have an Electrocardiogram (ECG) screening before participation. This applies to students with no cardiac symptoms. Students with cardiac symptoms should consult their healthcare provider. An ECG completed within two (2) years prior to July 1 of the participation year satisfies this requirement.

    EL01 - ECG Form

  • A sports physical form, also known as a Pre-Participation Physical Evaluation (PPE), is required before students can join school sports. This form helps confirm that your child is healthy and ready to safely participate in athletic activities. It also helps identify any medical concerns that may need attention. Most schools and sports leagues require a completed sports physical before the season begins.

  • The Florida High School Athletic Association (FHSAA) requires the EL3 form, officially known as the Consent and Release from Liability Certificate, for student-athletes participating in interscholastic athletics in Florida high schools.

Optional Forms (Based on Student’s Situation)

  • The EL7 form is a Florida High School Athletic Association (FHSAA) registration form for home education students who wish to participate in interscholastic athletics. It serves to verify the student's home education registration and eligibility for participation in FHSAA-governed sports. Essentially, it's a way for home-schooled students to register with the FHSAA and confirm they meet the necessary requirements to play sports at a member school.

  • Notice of Intent of Home Education Program form, which is used in Florida to formally notify the local school district that a parent or guardian is establishing and maintaining a home education program for their child.

  • The EL15 form is a Florida High School Athletic Association (FHSAA) form used for Participation in Sports for PEP Students (Personalized Education Program). The EL15 is for students who are enrolled in a private or charter school and wish to participate in athletics at a public school. The form requires the student, parent/guardian, and a notary public to complete and sign it.

  • A student may not participate in an athletic activity of this Association unless he/she is an amateur. An amateur is one who engages in athletic competition solely for the physical, mental, social, and pleasure benefits derived from the activity.

    GA1 - NIL Affidavit of Amateurism

  • A GA04 form, also known as an Affidavit of Compliance with the Policies on Athletic Recruiting & Non-Traditional Student Participation, is a document used in Florida high school athletics. It's primarily required for students who transfer to a new high school, especially if they didn't start their 9th grade year at that school. The form serves to verify that the student's transfer is not due to athletic recruitment and that the student meets the eligibility requirements for participation in sports.
